here’s how it works:
1. you pay what you will for the custom poem. (minimum $10)
2. you fill in the form below, ensuring i have all the info i need to make the poem happen.
3. i type your completely original poem on a beautiful piece of paper, and mail it to the person of your choosing.

please fill the form in entirely. if you have any special requests, notes, stories you must tell, please add your comments to the subject field at the bottom of the form.

NOTE: if you’ve ordered more than one poem, please tell me how many poems you’ve ordered. you are welcome/encouraged to place all of your orders on the same order form. just make sure you include all of the required info for each poem (who it’s for, where to send it, who it’s from, whether or not the sender should be confidential, and what the poem should be about).
